Dental Plan Services
- Exams: 1 comprehensive & 1 periodic per benefit year
- Cleanings: 2 per person per year (in absence of periodontal disease)
- X-rays: 1 recall set per year, 1 full mouth series per every 3 years
- Flouride Treatments: 1 per person per calendar year
- Oral Cancer Screening: 1 per person per calendar year
- 15% off regular fees for general dentistry (excluding Invisalign clear braces)
Annual Dental Plan Membership
- Individual: $399
- Individual and spouse/partner: $699
- Each additional family member: $325
